Couch R-1's Health Practioner, Amelia Crowder, is excited to announce Couch School will be hosting free sports physicals and immunizations for any Couch student on June 28th in the Old Gym. Sports physicals will be from 8 a.m. to 5 p.m. and immunizations will be from 9 a.m. to 2 p.m. Please click on the signup genius link below to schedule a physical or immunization. Physical forms can be found in the high school office. If your student will be attending summer school, you will not need to schedule an appointment for a physical as we will work those in throughout the day. Any student needing immunizations will need to schedule an appointment. This will be the only time we have free sports physicals this year.
